Injury & Incident Reporting requirements

In order to start the worker’s compensation process, the two injury report forms below should be submitted to hr@cals.wisc.edu.

Note: there is a Non-Employee Incident Report Form that should be completed when a non-employee is injured

When UW-Madison staff or faculty are informed of incidents involving property damage or personal injury of an individual, please notify the Office of Risk Management immediately (within 48 hours). Incident forms are intended to be completed by UW-Madison employees based upon information obtained from the persons involved in the incident. Once submitted, the form will be automatically sent to the office of Risk Management at riskmgmt@wisc.edu, and you will receive an email copy of the report. Injury & Incident Reporting Link
